Fall Sports Physicals/Information
2011-2012 Physical Information
If a student had a physical done at school during the summer or by his/her family doctor that is dated after June 1, 2011, and this is their first sport for this school year, he/she is cleared to participate in a fall sport. If he/she did not get a physical, it will be the parents/guardians responsibility to schedule a physical with their family doctor or you can go to Premier Urgent Care in Douglassville. They are open 9AM to 9PM everyday, with the physical costing $25.00. All forms are located on the school district’s website under Our Schools and then Athletics. Forms are also available at the junior high office. Athletes cannot participate until all forms are received.

Recertification Information
A student who participates in more than one sports season in a school year and who got a physical at their family doctor will be required to complete the Recertification Form (found below). The only other required forms are the Emergency Form and the Eligibility Form (found above).
