IMMUNIZATION CHANGES - Effective 8/1/11
PARENTS-PLEASE NOTE: NEW IMMUNIZATION REQUIREMENTS GOING INTO EFFECT FOR THE 2011-2012 SCHOOL YEAR (Effective 8/1/11).
The new immunization laws have gone into effect. Notices will be going out October 28, 2011 if your child does not meet the requirements listed below. If your child does not meet these requirements, he/she is provisionally enrolled for the 2011/2012 school year. Failure to provide proof of immunizations from your doctor will result in exclusion from school after April 30, 2012.
Attention Parents/Guardians DON’T WAIT -----------VACCINATE NOW FOR ATTENDANCE IN ALL GRADES SCHOOL REGULATIONS in 2011/2012 children need the following: 4 doses of tetanus* (1 dose on or after the 4th birthday)
4 doses of diphtheria* (1 dose on or after the 4th birthday)
3 doses of polio 2 doses of measles** 2 doses of mumps** 1 dose of rubella (German measles)** 3 doses of hepatitis B 2 doses of varicella (chickenpox) vaccine or history of disease
*Usually given as DTP or DTaP or DT or Td **Usually given as MMR
Children ATTENDING 7th grade in 2011/2012 need the following: 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) (if 5 years has elapsed since last tetanus immunization) 1 dose of meningococcal conjugate vaccine (MCV)
These requirements allow for medical reasons and religious beliefs. If your child is exempt from immunizations, He/she may be removed from school during an outbreak. 11th GRADE PHYSICALS (CLASS OF 2012) All 11th grade students are mandated by the State of PA to have a physical examination. You have the option to have this exam done by your private doctor or by the school doctor. Any physical exam done since September 2010 will qualify as the 11th grade physical for students in the graduating class of 2013. If you choose to have the exam done by your own doctor, the deadline to return the completed Private Physical Exam form was SEPTEMBER 15, but the nurse continues to accept these forms. Contact the BASH Health Office to pick up a form or go to the Boyertown Website (www.boyertownasd.org), and download the physical form through this link – Physical to obtain a form.
Any student who does not have an 11th grade physical exam form on file in the Nurse’s Office will be required to have the exam done by their own doctor since the school doctor has completed the school exams. Failure to return the form will result in lost privileges
Please make a copy of the private physical exam form prior to handing in to school.
TENTH GRADERS AND PHYSICAL EXAM FORMS (CLASS OF 2013) All tenth graders will receive the Private Physical Exam form via mail. The physical examination is a requirement mandated for all 11th grade students residing in PA. Any private exam done within the year prior to the start of 11th grade qualifies for the mandate. Therefore, the nurse is accepting any private exam done after September 1, 2011 for a tenth grade student in the 2014 graduating class. Many 10th grade students go to their doctor to have a physical for working papers or a driver’s permit. For these appointments, please take the Physical Examination form with you and have it completed. Remember to make a copy of the private physical exam form prior to handing in to school.
EMERGENCY HEALTH FORMS
The Secondary Emergency Health Office form was due the first week of school. Several students still have not returned the Secondary Emergency Health Office form. If your child has not brought his/her form in, please make sure he/she brings it in immediately. If you have misplaced the form, or do not recall getting it in the mail, have your child stop by the Nurse’s Office to have a new one printed out. Disciplinary action will be taken for students who have not submitted this form.
MEDICATION ADMINISTRATION
If your child needs to take medication during the school day, either prescribed, over the counter, herbal or vitamin supplements, this must be done through the Health Office (please see the student handbook). Students are not permitted to carry and self-administer medications. A Request for Medication Administration form needs to be completed by both the parent and the prescribing doctor. Forms can be obtained through BASH Health Office or via the Boyertown Health Services Website (found in student services) (www.boyertownasd.org) or through this link – Medication Form
CELL PHONE USAGE AND GOING HOME POLICY
Remember that if your child is ill or has an injury they should visit the School Nurse. If the parent needs to be contacted or the student needs to go home, this will be done at the Nurse’s Office. No student is to leave the school building for the reason of illness or injury without clearance from the nurse.
USE OF ELEVATOR
ATTENTION ANY STUDENT NEEDING TO USE THE ELEVATOR DURING THE SCHOOL YEAR. Please check the student handbook if you are unable to climb the stairs due to a medical injury or illness. In such situations, you may use the elevator if the following procedures are followed: 1. A note from your doctor indicating the length of time the elevator will be required. 2. A $5.00 deposit that will be refunded when the elevator key is returned. Report to the nurse with your note and $5.00 to obtain the elevator key. You will also be issued a note giving permission to leave class early. Once the key is provided, only the student requiring the use of the elevator is permitted to ride on it and to leave class early. Helpers are not to be riding the elevators or leaving class early.
