Annual 1st Day Student Paperwork 2025-2026: Below are all the forms distributed to students on their first day of school. Some of these forms are sent home via paper copy. Some are sent home electronically, and some are included in our online Infinite Campus Student Registration/Annual Student Information Verification (ASIV). Additionally, some of our schools have school-specific forms.

A response is required IF your child receives medication at school or IF one of the exams is required for their current grade level.

Technology Usage Fee Letter
Grades 2 - 12
A reminder message has been added to ASIV. An email will go out for payment due in October.
No response required as part of ASIV. Payment is required in October.
Payment will be due through EZSchoolPay or via check/money order in October 2025.
If paying by check or money order, please pay to Boyertown Area School District.

Activity Fee Letter
Grades 7 - 12
A response and payment are required for middle and high school students participating in athletics, music programs, and other activities. Students will be charged a fee on their Infinite Campus accounts once rosters are set following the start of the appropriate sports/activity season. PLEASE DO NOT PAY BEFORE YOU ARE BILLED IN INFINITE CAMPUS.**
**Students who receive free or reduced lunch are exempt from the Activity Fee. A copy of the district's reduced/free lunch letter must be sent to the Athletic Office with this completed form.
CHIP Flyer
Applies to All Grades
This flyer provides the information needed to apply or renew with CHIP - Pennsylvania's Children's Health Insurance.

Included in the annual back-to-school newsletter.
Food Services Information Packet
Applies to All Grades
A response is required for the Free & Reduced applications or if creating an EZSchool Pay account.
Integrated Pest Management Letter (IPM)
Applies to All Grades
A response is optional for families who want their child(ren) listed on our notification registry.
A paper copy or IC email is sent home on the first day of school.
