    Digital Backpack

    Annual 1st Day Student Paperwork 2022-2023

    New for the 2022-2023 school year, we are providing parents/guardians and students with a digital
    backpack containing all of the forms distributed to students on their first day of school.

    Some of these forms are sent home via paper copy.  Some are sent home electronically, and some
    are included in our online Infinite Campus Student Registration/Annual Student Information
    Verification (ASIV).   Additionally, some of our schools have school-specific forms.

     

    • Acceptable Use Policy Sign-off
      Applies to All Grades
      A response is required as part of the Annual Student Information Verification (ASIV) in Infinite Campus (IC).
      ASIV is open from August 1 through September 10, 2022.

    • Activity Fee Letter
      Grades 7 - 12
      A response and payment are required for middle and high school students participating in athletics, music programs, etc.
      Sent via IC email to grades 7-12 families on July 25, 2022.

    • CHIP Flyer 2022-2023
      Applies to All Grades
      This flyer provides the information needed to apply or renew with CHIP - Pennsylvania's Children's Health Insurance. 

    • Free and Reduced Lunch and Annual Household Letter Mailing
      Applies to All Grades
      A response is required for families who qualify for free or reduced lunches.
      Sent via IC email on July 27, 2022.
       
    • Fundraising Sign-off
      Applies to All Grades

      A response is required as part of ASIV in IC.

      ASIV is open from August 1 through September 10, 2022.

    • Integrated Pest Management Letter (IPM)
      Applies to All Grades
      A response is optional for families who want their child(ren) listed on our notification registry.
      A paper copy or IC email is sent home on the first day of school.

    • Limerick Letter
      Applies to All Grades
      A response is required for families living within the required boundaries.
      A paper copy is sent home on the first day of school. 

    • Lunch Packet from Food Services
      Applies to All Grades
      A response is required for Free & Reduced Lunch applications or if creating an EZSchoolPay account ONLY.
      Sent home via Back-to-School Newsletter on August 16, 2022.

    • Publicity Sign-off
      Applies to All Grades
      A response is required as part of ASIV in IC.
      ASIV is open from August 1 through September 10, 2022  

    • Student Handbook Sign-off
      Applies to All Grades
      A response is required as part of ASIV in IC*.
      ASIV is open from August 1 through September 10, 2022.
      *BASH also requires a paper sign-off.  A paper copy is sent home with students on the first day of school.

    • Student Health Services Forms
      Applies to All Grades and includes Dental Exam, Physical Exam, and Medication forms.
      A response is required if your child receives medication at school or if one of the exams is required for their current grade level.

    • Tech Usage Fee Letter
      Grades 2 - 12
      A response and payment are required.
      A paper copy is sent home with students on the first day of school.
      If paying by check, please pay to Boyertown Area School District.

    • Voluntary Student Insurance
      Applies to All Grades
      A response is only required if a parent/guardian would like insurance for their child.
      Included in Back-to-School Newsletters on August 16, 2022.
